The names below are a list of deceased persons whose group life insurance benefits
are payable to the beneficiary determined under Wisconsin Statute
40.02 (8)(a) upon application and satisfactory proof of entitlement.
Application forms and information may be obtained by writing the
Department of Employee Trust Funds, PO Box 7931, Madison, WI
53707-7931. Inquiries should include the participant's complete
name (including published names and former names(s)), date of birth,
Social Security Number and year the name was published.
